ENROLLMENT REQUIREMENTS
- Original and photocopy of Official transcript of Records
- Certificate of Graduation
- Certificate of Good Moral Character
- Photocopy of PSA birth certificate
- 2x2 ID picture
- Affidavit to use married name ( for married female students)
- PSA Marriage Certificate (for married female students)
- Original and photocopy of the documents should be placed in a long white folder.
ENROLLMENT PROCEDURE
- Prepare the original and photocopy of all admission requirements
- Visit the respective Dean's office and present a copy of your grades for evaluation. If approved, you will receive an admission slip signed and approved by the Dean or School Secretary. After the evaluation, have the evaluation form photocopied for your records
- Register at UB Student Portal
- Click “Apply as a new student”, complete all the needed information and submit the registration
- Fill-out the necessary information in the Pre- registration Google Form
- Fill- out the Declaration, Waiver and Consent Agreement Form
- Submit the Original and photocopy of admission requirements including the photocopy of your evaluation to the admission officer for scanning and verification
- Proceed to the Dean’s Office for the encoding of courses based on the results of the evaluation
- Pay the required down payment (35%)online or at the Cashier's Office.
- Present your receipt to the Student Accounts Office for ID validation slip
- Proceed to MIS office and present your ID validation slip for ID application
- Check your schedule on the Student Portal
