Reservation of Slots for New Pupils, Transferees, and Returning Students

đź“ť Reservation Process

  1. Register at https://portal.ubaguio.edu
  2. Click “Apply as a New Student”, complete all the required information, and submit the registration.
  3. You will receive an email containing your Applicant ID Number, which will be used to pay the Reservation Fee.
  4. Upload the proof of payment and scanned requirements through this Google Form: https://forms.gle/nN3P1NYxsPDdKftn6
  5. After submitting the reservation form, wait for the schedule of enrollment for new and transferee students to be posted on the UBLES Facebook Page. (https://www.facebook.com/profile.php?id=61580307134883)
  6. Proceed to UBLES on the scheduled date for face-to-face enrollment and bring the complete requirements.
    ⚠️ Students will only be enrolled once all requirements are complete.

Nursery and Kindergarten
• Photocopy of Valid ID of Guardian
• One (1) 2×2 picture of the pupil
• Two (2) photocopies of PSA Birth Certificate

Grade 1
• Photocopy of Valid ID of Guardian
• One (1) 2×2 picture of the pupil
• Report Card / SF9 (Original and Photocopy)
• Two (2) photocopies of PSA Birth Certificate
• Form 137 (Original and Photocopy)
• Good Moral Certificate
• Certificate of Kindergarten Completion (for incoming Grade 1)

Grades 2–6
• Photocopy of Valid ID of Guardian
• One (1) 2×2 picture of the pupil
• Report Card / SF9 (Original and Photocopy)
• Two (2) photocopies of PSA Birth Certificate
• Form 137 (Original and Photocopy)
• Good Moral Certificate
