For University of Baguio Grade 12 Students expected to graduate at the end of the current School Year
ONSITE RESERVATION ONLY
RESERVATION REQUIREMENTS
- 1 Photocopy of PSA Birth certificate
- SF9/ Grade 12 Report Card First Semester grades (Photocopy only)
PROCEDURES:
- Download and fill up the Certificate of No Failed Grades and have your respective Principal verify and sign the form.
- Pay the reservation fee of Php 2,000.00 at the UB Cashier Office
- Visit the Admission and Records Center (ARC) and submit the printed reservation requirements to the admission officer for scanning and verification
- Fill-out the necessary information in the Pre - Registration Google form that will be provided by the Admission Officer
Note:
To complete your reserved courses to enrolled status, submit the original F138/SF9 (Grade 12 Report Card) and other requirements that were not submitted during the reservation period.
Reminders:
- Reserved slots is valid until (June 26, 2026)
- Unconfirmed slots shall be given to those with complete enrollment credentials
- Visit the ARC as soon as all credentials are available
- Please inform the office if you plan to enroll in classes for SHORT TERM 2026
