📘 INCOMING GRADE 7 REQUIREMENTS

• Original and two (2) photocopies of Report Card Back-to-Back (School Form 9)
• Three (3) photocopies of PSA Birth Certificate
• Three (3) copies of a recent 2×2 ID picture of the student

📌 ESC APPLICATION REQUIREMENTS

• Latest Income Tax Return or Certificate of Tax Exemption from BIR
• ESC Application Form and ESC Grantee Enrolment Contract
(To be given to the applicants once all requirements are completed)

🎓 INCOMING GRADE 11 REQUIREMENTS

• Photocopy of Certificate of Junior High School Completion
• Photocopy of PSA Birth Certificate
• One (1) recent 2×2 ID picture of the student

📌 Important:
Students without ESC Grant in JHS must apply for the SHS Voucher through the PEAC Online Voucher Portal.
If the application is APPROVED, download the QVAC and submit a hard copy.

🔬 SCIENCE HIGH QUALIFYING ASSESSMENT (SHQA)

✔ Qualifications

Applicants must:
• Be a currently enrolled Grade 6, Grade 7, or Grade 10 student
• Have an average grade of 80 in all subject areas for SY 2024–2025
• Have no grade below 80 in any grading period for SY 2025–2026
• Have no major school violations in the current and previous school year
• Be physically, emotionally, and mentally fit to take the UB Science High School Challenge

📋 SHQA Requirements

• Copy of grades from the current school year (SY 2025–2026)
• Copy of complete grades (1st–4th grading) from the previous school year (SY 2024–2025)
• Complete name and email address of the recommender from the current school
(Adviser, Principal, Subject Teacher, or Counselor)
• Recent photo of the applicant
• Certificates of awards (academic and non-curricular) received in the last two years
• Receipt of SHQA processing fee – Php 300.00

📝 SHQA Assessment Procedures

1️⃣ Prepare all application documents.
2️⃣ Download and process the Science High Qualifying Assessment Recommendation Form.

3️⃣ Fill out the Online Application Form.
4️⃣ Wait for your Applicant ID Number.
5️⃣ Attend the Examination Orientation.
6️⃣ Take the Science High Qualifying Assessment.
7️⃣ Attend the Online Interview.
8️⃣ Release of SHQA Results.
