Transferees Students with Special Needs

ENROLLMENT REQUIREMENTS

  1. Accomplished Student Directory with Declaration, Waiver and Consent Agreement Form
  2. Accomplished First Semester Registration form SY 2026-2027
  3. Transfer Credential with copy of grades
  4. Certificate of Good Moral Certificate
  5. Photocopy of PSA Birth Certificate (If born abroad, submit a photocopy of the Philippine Passport bio-page)
  6. Photocopy of PWD ID (back-to-back)

PROCEDURES

  1. Applicants are required to consult with the person- in- charge at the Center for Counseling and Student Development (F206), once done with the consultation, wait for an endorsement form.
    1. This step allows the University to assess individual needs and ensure that proper support and accommodations are provided.
  2. Complete the application form available online at UB Student Portal make sure to indicate Disability on the Application Form, please fill out the section labeled “Specify the disability.”
  3. Accomplished Student Directory with Declaration, Waiver and Consent Agreement Form or obtain a physical form at the Center for Counseling and Student Development (CCSD) at F206, please fill out the section labeled “Specify the disability.”
  4. Submit the Original and photocopy of admission requirements including the photocopy of your evaluation to the admission officer (ARC OFFICE) for scanning and verification
  5. Fill-out the necessary information in the Pre-registration Google Form 
  6. Proceed to the Dean’s Office for the encoding of courses based on the results of the evaluation
  7. Pay the required down payment online or at the Cashier's Office.
  8. Present your receipt to the Student Accounts Office for ID validation
  9. Proceed to MIS office and present your ID validation for ID application
  10. Check your schedule on the Student Portal
