For Grade 12 Students expected to graduate at the end of the current School Year

REQUIREMENTS AND PROCEDURES FOR RESERVATION OF SLOT

REQUIREMENTS:

  1. Certificate of No Failed Grades

2. Certified true copy Grade 12 Card (First Semester grades F-138/SF9) duly signed by the school principal. 

  1. Photocopy of PSA birth certificate

NOTE: Students born abroad should submit a photocopy of Philippine passport (BIO-PAGE) or Philippine Recognition Certificate.

REQUIREMENTS FOR TRIMESTER SCHOOL

  1. Certificate of No Failed Grades

2. Certified true copy Grade 12 Card (First Trimester and Second Trimester grades F-138/SF9) duly signed by the school principal. 

  1. Photocopy of PSA birth certificate

NOTE: Students born abroad should submit a photocopy of Philippine passport (BIO-PAGE) or Philippine Recognition Certificate.

ONSITE RESERVATION

PROCEDURES

  1. Register at UB Student Portal

Click “Apply as a new student”, complete all the needed information. Once complete, click submit the registration. You will receive an email Containing your APPLICANT ID NUMBER that you will use for the reservation fee

2.Download and fill up the Certificate of No Failed Grades and have your respective Principal verify and sign the form.

3.Submit all reservation requirements to the admission officer and wait for the instruction and Student ID number.

4.Pay the reservation fee of Php 2,000.00 at the UB Cashier Office

5.Present your receipt to the Admission officer for slot registration

6.Fill-out the necessary information in the Pre - Registration Google form that will be provided by the Admission Officer

ONLINE RESERVATION

PROCEDURES

  1. Register at UB Student Portal

Click “Apply as a new student”, complete all the needed information. Once complete, click submit the registration. You will receive an email Containing your APPLICANT ID NUMBER that you will use for the reservation fee

2.Download and fill up the Certificate of No Failed Grades and have your respective Principal verify and sign the form.

3.Pay the reservation fee of Php 2,000.00 thru accredited banks or at the UB Cashier Office (PAYMENT PROCEDURES)

4.Prepare and scan all the reservation requirements (PDF Format Only)

  1. Fill-out the necessary information in the PRE- REGISTRATION GOOGLE FORM

Note:

To complete your reserved courses to enrolled status, submit the original F138/SF9 (Grade 12 Report Card) and other requirements that were not submitted during the reservation period. 

Reminders:

  • Reserved slots is valid until (June 26, 2026)
  • Unconfirmed slots shall be given to those with complete enrollment credentials
  • Visit the ARC as soon as all credentials are available
  • Please inform the office if you plan to enroll in classes for SHORT TERM 2026
